If the applicant is under 18 years of age, they must provide:

Evidence that every person with a legal right to decide where the applicant lives consents to their proposed studies in Australia. (You must download the Under 18 Consent Form and ask the applicant to complete it);

Applicant's birth certificate;

Parent(s)'s passport or identification card.
Additionally, the applicant must provide evidence of one of the following:

If the applicant intends to reside in Australia with a parent or a person who has custody of them - evidence of their relationship and that they hold, or are applying for, a Student Guardian Visa for the length of the applicant's stay or until the applicant turns 18, whichever is the lesser period. (You must download the Under 18 Welfare Form and ask the applicant to complete it ); or

If the applicant intends to reside in Australia with a relative nominated by a parent or a person who has custody of them - evidence of their relationship and that the relative is at least 21 years of age, has a police clearance, and has the right to remain in Australia for the period of the applicant's study in Australia, or until the applicant turns 18, whichever is the lesser period. (You must download the Under 18 Welfare Form and ask the applicant to complete it.)
Note: Police clearances must be provided by the applicant's relative for every country in which they have resided for more than 12 months in the past 10 years (after the age of 16). Details on how to obtain a police clearance can be found in Form 47P Character Requirement: Police Certificate; or

If the applicant will not be residing in Australia with one of the above-mentioned persons - a signed statement from their education provider confirming appropriate arrangements have been made for their accommodation, support and general welfare during their stay in Australia.
If the applicant has family unit members who want a visa to travel with them to Australia:

Evidence of the relationship between the applicant and all family unit members (eg birth certificate, marriage certificate etc); and
-
If any family unit members are 5 - 17 years of age (inclusive) and will live with the applicant in Australia, provide documentation confirming their enrolment in an Australian school.
Health requirements
The applicant and any family unit member wanting a visa to
travel with them to Australia must undertake medical and x-ray examinations, if
they are requested by the system, or they receive a follow-up request from a
processing officer, to undergo these examinations.The applicant and any family
unit members should present their health forms (issued by TTS) to the authorised
Panel Doctor /
Radiologist, with their passport, when they undergo their medical
examination.
You do not have to copy or hold these health forms.
Student
Visa Application and Information Forms
Students Applications - to be completed by
applicant /agents and lodged with TT Services (TTS), the Reliable Business
Partner engaged by the Australian High Commission, New Delhi. Applicant /Agents
must ensure that the application is valid i.e. that the correct form is complete
and signed, and passport photo, fee draft, letter of offer or confirmation of
enrolment, agent declaration and financial matrix are completed and attached.
TTS will then issue medical forms and a receipt prior to lodging with the High
Commission the next day. The High Commission will issue an acknowledgement
letter within 72 hours advising the application has been received and is being
processed, and detailing the next steps in the process. Medicals will only be
accepted if they have been stamped by TTS.
The demand draft for the visa
fee is to be made out to the Australian High Commission payable in New
Delhi. TTS charge a service fee of Rs386 (approx AU$11.50) per application,
payable at time of application.
